If we are a provider for your plan, we will file insurance
for you so that we may be reimbursed according to your plan guidelines. If
we are not a provider for your plan, payment is due at the time of
service. We will provide you with all the information you will need to
file your claim and get reimbursed.
If you are not a United States resident, payment is due at
the initial time service is rendered.
